I, the shipper, certify the particulars given in this form are correct. This form does not contain any undeclared dangerous articles, or articles prohibited by Legislation or by postal or customs regulations. I have met all applicable export filling requirements under federal law and regulations. Narcotics, hazardous drugs or materials, handguns, flammable radioactive, alcohol beverages, animal, life plants, effusion, and motor vehicle or parts are items which can detain fluency of shipment and check up in custom or toll, and which can result in confiscation. Maximum claim for loss or damage of shipment per shipper is $400. Extra Insurance can be purchased for a fee of 7% from “insured for”. Extra charge from customs that belong to shipper’s product because of the undeclared items, will be imposed
